Fill out the first page of the form. It asks for your name, address, phone number, date of birth, race, and your highest level of education completed. The page will also ask if you are a U.S. citizen, disabled, or a veteran.

Then Answer the questions about your unemployment applications over the past 12 months if there are any and whether you are a member of a union. Types of employers include private companies, state or local government, federal government or military service.

Then list all of the employers you have worked for in the last 18 months. If any of the companies you worked for are in the common employer list, add them through that function. If not, type them in manually.

Answer all questions asked about the employers you listed, and then hit the "Click Mouse Here To Proceed" button at the bottom of the page.

Answer questions about your eligibility. These questions ask things such as you ability to start work, your ability to transport yourself to work, if you have turned down any work.

Hang on to the confirmation number you are given. You will be contacted within seven days and told whether you are eligible for unemployment benefits.
